For anyone out there like me who is reading this post just because they want to get rid of the "checkout" button on the cart page (and have paypal express only), I just turned off all other payment methods, leaving only paypal express (under Configure > Payment Methods), and voila - on my cart page there is now only a paypal button - no more checkout button.

I have never set up any custom buttons.
